Long hours

life is crawling too fast. it now seems like a remix of several tracks. downtempo/lo-fi with psytrance. some moving too fast and others too slow. sometimes leaves you wondering what is to come of all this.

over the past few weeks, sold my laptop for paltry sum and got my self a decent desktop computer with the regular. looks like a pentium d 2.66 ghz 1 gb ram 160 gb hard drive and a dvd burner. the best part is the 17" lcd i got from samsung. stunning. i was and am really happy. at first thought it was too big but got used to it now. it is good. through with my first dvd worth of downloads. lots of good music. can't really get over armin van buuren's state of trance episodes and some really good chill out and lounge music. its just awesome. in rock, got this album caled the carnival of rust by the poets of fall. decent piece. looking desperately for an album by aphelion called amethystium. some great tracks on there.

almost through with pediatrics rotation. the last two days left and i have already taken my ward exam. if there was something worse than pathetic, it was me. had a tough time. barely into final year and the professor expected me to know all way more than what i did. he wanted the entire history of renal disease right there in front of him. anyways the cat got my tongue and the dog my pride. i just sat there as he just went on and didnt utter a single word. well may be i would have done better if it was later on in the year when i was through with a major portion of the texts. it is obgyn for me next and hope to make good use of that. better than what i have done so far.

sprained my right ankle really bad. soft tissue injury plus some joint effusion was what my professor finally arrived at after considering intially a fracture base of fourth metatarsal. painful. been out of action for a week now. oh that fateful saturday when we decided to kick around with the ball. anyways it is rest for a couple of weeks now. just when i thougth people were finally catching onto the sport i liked.

there is finally a mcdonald's in hyderabad. after several years, the grease-burger-famished can now find salvation. i am waiting to gorge that junk. going there next week. visit family and spend sometime there.

Study of Electrolyte Imbalances in patients undergoing treatment for cardiovascular diseases

Cardiovascular diseases are one of the leading causes of mortality among populations of all countries of the world. The heart continues to bear the brunt of changing lifestyles with trends only showing a decline in healthy living. Though modern medicine has been successful in controlling diseases like rheumatic heart disease and bacterial endocarditis through proactive community based interventions and advent of newer drugs, the focus has now shifted to the modern epidemics of atherosclerosis, coronary heart disease, myocardial infarction and stroke. The emphasis on cardiovascular medicine and its implications is ever increasing in the search for novel drugs and treatments that can alleviate disease and improve prognosis for patients with these life-threatening conditions. Newer treatments are being developed by the day and present universal hope to both patients and doctors. But as with most therapeutics, the accompanying disturbances in the internal milieu of human physiology are not completely understood. Latest research continues to shape treatment protocols as a result of past experiences with the newer drugs. In this study an attempt is made to understand the imbalances in the all-pervasive electrolyte equilibrium in the body occurring as a result of medical intervention in cardiovascular disease.

Aims & Objectives
To study the levels of various electrolytes in patients undergoing treatment for cardiovascular diseases.
To study and correlate these levels with any predisposing factors present in these patients.
To find, if any, the impact of this electrolyte status on the outcome of treatment and prognosis for the patient.

Material and Methods

A sample of 50 patients admitted to the cardiology and medicine wards for cardiac ailments will be studied over a period of 2 months. A standard pro forma will be designed to elicit the necessary information like age, sex, occupation, education, habits like drinking and smoking, emotional stress factors etc and all the relevant information. All the routine investigations will be studied in addition to the serum electrolytes, which include serum Na+, K+, Ca++ and Mg++ etc.

Type of Study

The study proposed is a Clinical study.

Analysis & Implications of Study

Many of the drugs used in treating cardiac disease work directly through interference with cellular electrolyte transport systems. For example digoxin works by increasing the intracellular Ca++ thus enabling the positive inotropic effect while furosemide increases loss of Cl- anions to enable diuresis. In this study, an attempt is made to correlate the change in levels of some electrolytes with that of others and their bearing on the overall progress of disease as well as treatment. The data collected will be subjected to suitable statistical analysis with an attempt to identify, if any, departures from the normally electrolyte changes. Any changes thus found will suggest scope for further research to understand possible treatment modifications that will preserve electrolyte balance.

Good news, Bad news

they say an increasing number of foreign patients will be headed to indian shores for treatment that is cheap and compares well with that received in their home countries. this means there are good growth opportunities for corporate medicine. this reflects the world-class quality of indian healthcare providers. but quite unfortunately none of this advanced medical care will be available to the average citizen of the country. all the foreign exchange goes into private coffers or indifferent government treasuries. someday the common man can hope to receive the same treatment meted out to the cash heavy foreigner. the outcome of this business is an increase in posh hospitals and swanky private clinics while the need of the hour is decent rural health centres. well hope is that somewhere along the line the doctors, the businessmen or the government will realise the bare necessities of the populace and will strive to seek a balance between business and responsibility.

bbc reported that m f hussain's paintings have again caused a furore and hindu nationalist protests have lead to an apology and withdrawal of the painting from an auction to help the victims of the recent natural disaster.

whoever these hindu nationalists are probably have never been to a hindu temple. most of the edifices are decorated with sensual depictions of the human form which are supposedly some of the numerous gods and goddesses. khajuraho is my answer to these people whoever they are. the temple is as hindu as it gets but is famous for the evocative and erotic culture. maybe the hindu nationalists need to go figure their idealogies or simply get a life.

Well i guess indian engineers can now get busy at home. i just hope this wont spur the growth of new colleges. knowing things here, soon there may be an engineering college in every village. 258 already!!

What?

well i was reading some of my own posts and yeah i know what you feel like. anyways i liked it and like it. blah-ing away at odd hours to an audience who will always listen and having the advantage of getting numb fingers instead of a sore throat. its good. sense whether to you it makes or not is inconsequential.

though it is truly amazing what the mind spits out when you are bored in the middle of the night.

To the Moon and Back!

hmmm. its been a while since i cared to update this space. been through exams in december and then went to kuwait in january. the trip was amazing. i met everybody after a long while and it was definitely worth it. all the others graduate next may and move onto jobs and stuff.

i dont know but i kinda felt irresponsible and felt the pressure build, to go on and be something but i guess there is still time for me so i shouldnt really worry from now. from what i hear thats what i will be doing for the rest my life anyway. well it was an experience though as it helped me put somethings in perspective. i got a few things cleared up in life. i was really glad to have the chance to step out and refocus on the issues at hand than all the other bullshit i keep thinking of doing.
but there is still more i need to learn and time is short. then there will soon be a crush in final year when burden will increase and it will help to have the focus gained.

but i already feel the pep fading. the bloody atmosphere here does it to you. lets you wander aimlessly. therefore more and more energy is spent on useless pursuits. these days surfing the cyber waves and cricket. it is 4 am and what am i doing up. well time is abundant and there is no necessity to work. end result? layers of fat (physical and mental). i have got to change this and find a feasible routine. hahaha. yeah right. three and half years it has taken me and i doubt i will find it now. i will retire for the night or rather morning shall we say.

For What It Is Worth : Love

most of us at some point or the other in our lives are initiated into the world of love. for most it is the fluctuant adolesence that brings the spring of this new found emotion. others are a little late. but nonetheless it is assured that all of us know that we have been or will be there.

love, it should be agreed, is the one of the most overrated, overused, least understood words of our time. for different people it elicits diverse emotions. as diverse are the varieties that are out there for you to get. all you need to do is ask. there are the traditional varieties like motherly and fatherly loves, the sisterly and brotherly loves, and there are also in the same family, products of more exotic kinds like the sexual love, platonic love and spiritual love. if only it could be put in a box, walmart would have made a killing. $50 a kilo. well the way the world is headed, the days aren't too far away when people will bid to buy love on ebay. and some of us cant wait for that!!

but what is this word that we throw around? is it a sin to use it in vain (alongside the lord's name)? should there be a code for its usage? is it guaranteed by law? foolish as it may seem, this thing(love)can cause more agony than george w.'s union address and more joy than cold water in the sahara. listed with the merriam-webster are more than 24 entries for love and they range from the motherly mushy definitions to the more exciting amorous sexual episodes. but what i found most apt was " love : 8. a score of zero (as in tennis) ". those four words captivate the beauty of it all with a simplicity only god and the guy who invented tennis could think of. love as a score of zero symbolises the state of nullification of the intellect, the senses, the wisdom and not the least the freedom of the individual spirit. a little harsh? well, bluntly put people in love make stupid decisions(and you thought it was only in the movies). we can lie but cant deny the truth. we have all been there and those that have not as yet will so be there. and as for the senses, the diktat of love is such that for that rosy period of your life when you think you are in love, nothing can get better. yeah right! and finally all my fellow beings who have entered the bond of monogamous commitment to a fellow being will know that the freedom lost, though also in the sexual plane (homo erectus zindabad!), is more importantly of the emotional, private, social, professional and rarely spiritual kinds.

well all this said, is love a bad thing? let me characterise love for you. simply put, love is a syndrome not a single entity. it encompasses several mental and physical states which together lead to the more subjective notion. undeniably, it is first a physical attraction to a fellow being, therefore placing first the need for physical compatibility. the next step is that of personal compatibilty, the nuances of which have been elaborated time and again since the birth of society and its order. these include the usual social, professional, familial and the likes. these satisified does not yet constitute love. there has to be an essential commitment, no matter how brief, and an out-of-this-world feeling to go with it. tada! you are in love.

is it worth the trouble? well let me tell you, ideas like falling in love or that love just happens are far fetched. love or any such phenomenon is all about subconscious calculations based on comparisons of the like with like and a measurement of the unlike and how much of it one is willing to consider. this, for example, simply explains the success of arranged marriages. the couple have a wide bandwidth within which they are willing to accomodate the differences. this however is not universal. all rules have exceptions. other type of marriages, the love marriages, work a lot of the times too, provided that the participants in such a relationship subscribe less to romantic imagination and are adaptable and compromising enough to enable creation of a similar understanding. highly individual choices and independence bring with them the inevitable clash of personalities which is often shrouded by the cloak of love. once the rose coloured glasses come off, these formerly subliminal bombs surface to create all havoc.

life is all about getting what you want, making most of what's available and being content with what you have. these are the lessons learnt over millenia of civilisation which brought with them complexities of communal life, society and identity. an adaptable, understanding relationship based on mutual trust and respect with adequate scope for individual expression is the ideal foundation to experience the higher emotions of conjugation, cohabitation and coexistence. two voices come to sing as one, each retaining its unique beauty.
